Shot Records

Please ask the nurse for a copy of the shot record at any visit when your child is given shots.  This is a very important document that you will need to keep in a safe place since you may be asked for it by daycare, school, sports organizations, etc.  If you need to request a copy from our office, please phone 972.985.0381 and press the option for records.  There will be a $10.00 fee for this service if not processed during a  visit and shot records may only be mailed to you.  They may not be picked up at our office.**  After the fee has been collected, the shot record will be mailed within 7-10 working days.  EVERY EFFORT will be made to mail records sooner, however, certain times of the year require 7-10 working days.

School Sports and All Other Forms

School, sports and all other forms must be dropped off or mailed to our office with a self-addressed stamped envelope and the $10.00 fee.  Be sure that the form has your child's name and date of birth on it.  The form will be completed and mailed to you within 7-10 working days.  The form cannot be picked up,** it must be mailed to you.  Any form received without an envelope or without payment will not be completed and mailed. A copy of the shot record will be included with the form, unless otherwise requested.

**NDP recognizes that emergency circumstances arise that may not always allow the above guidelines to be followed.  To accommodate you when this happens, you may come to the Plano Office, Tuesday - Friday, between the hours of 10:00am and 12:00pm and 1:00pm and 3:00pm.  During these hours, you will be able to pay a $25 emergency fee and then wait while we prepare a copy of your child's shot record or complete the form for your child.  There will be NO exceptions to this policy for all emergency requests.
