TO LINK THE SIBLINGS ACCOUNT
a. After you have created an account for each child, log in as the oldest child and go to “My Patient Page."
b. Click on “Change My Family Account.” c. Click on "Link An Existing Family Member." d. Enter the User Name and Password for the sibling who is being linked to the oldest child's account e. Repeat steps a - d for each sibling. This allows you to access each child's account when you log in using your oldest child's User Name and Password. The benefit of this is that you will only need to remember your oldest child's User Name and Password. Please Remember to Print and Complete
ALL RELATED DOCUMENTS In Black Ink If you are a new patient and have CREATED AN ACCOUNT and have completed the Pre-Registration Form and Health History Form please print them and bring them with you on your first visit. Then, print the New Patient Package, complete, sign and bring these on your visit also. This should keep you from having to fill out any paperwork in the office. We will ask to make a copy of the Insurance Card and your Driver License.
If you are a new patient and do not choose to CREATE AN ACCOUNT through our secure web portal, print the file labeled General Info and History Forms as well as the New Patient Package. These are all of the forms that will need to be completed for your first visit to our office. This will get you out of our waiting room and back to see the doctor a lot quicker. We will still ask to make a copy of the Insurance Card and your Driver License. All of the forms are listed below and may be downloaded and printed with Adobe Reader. |
